efforts on the eve of World AIDS Day to pay tribute to those
currently living with HIV/AIDS, our fallen loved ones and the
unsung heroes of the AIDS Crisis; the doctors, nurses and caregivers
whose work all too often goes unnoticed.

If you are making a donation to honor a loved one, please include their name for inclusion in
the program. The deadline for this inclusion is two weeks before the program.

Please include your email address and phone number on any checks or money orders sent in order
for us to keep in touch.

Your privacy will be honored and we DO NOT sell, rent, loan, give or share any personal information.

Donations to Hollywood Remembers, a tax-exempt organization under Section 501(c)(3) of the Internal Revenue
Code, are deductible for computing income and estate taxes.
